Excellent customer service does not stop at the front door of Sherman Oaks Hospital, Encino Hospital Medical Center and Chino Valley Medical Center. Skilled nursing facilities, assisted livings, and board/care homes are critical partners in the health and well-being of many patients. Our Epic Medical Group Internists go out into the community to actively build relations with our continuum of care partners who play a vital role in a patient’s transition of care back to their home. Following a hospital stay, our Internists are available to continue following their patients and provide medical services in these facilities – delivering “continuum of care” services to patients and strengthening relations between patient, family, and the doctor. Our newly opened transitional care multi specialty clinic, led by Dr. Keino Rutherford provides our community with a trusted post hospitalization resource.
