Excellent customer service does not stop at the front door of Sherman Oaks Hospital, Encino Hospital Medical Center, and Chino Valley Medical Center. Skilled nursing facilities, assisted livings, and board/care homes are critical partners in the health and well-being of many patients. Our Epic Medical Group Internists go out into the community to actively build relations with our continuum of care partners who play a vital role in a patient’s transition of care back to their home. Following a hospital stay, our Internists are available to continue following their patients and provide medical services in these facilities – delivering “continuum of care” services to patients and strengthening relations between patient, family, and the doctor. Our newly opened transitional care multi-specialty clinic, led by Dr. Keino Rutherford provides our community with a trusted post-hospitalization resource.
